When Joy came in to CRUSA and had blood work,
it was discovered that she had a serious liver problem.
At first, it was thought that she had a portosystemic shunt (PSS),
also known as a liver shunt, which is a bypass of the liver
by the body's circulatory system.
Further tests at Michigan State University Veterinary Medical Center
ruled out a shunt, but it seems that poor Joy has a vascular liver problem.

The likely cause of Joy's elevated liver enzymes is a condition called
hepatic microvascular dysplasia (HMD), or portal vein hypoplasia.
This is a condition in which the very small blood vessels in the liver
do not form correctly and are either underdeveloped or not present at all.
This decreases blood flow within the liver,
causing a decrease in size of the liver and its cells.

The liver cannot appropriately process toxins or make the proteins
necessary for healthy growth and normal body functioning.
If toxins build up in the blood stream,
symptoms such as wobbly walking as if drunk,
staring into space, head pressing, abnormal vocalization,
and seizures can occur.
Other symptoms include loss of appetite, diarrhea,
or changes in urinary patterns,
including development of urinary stones or blood in the urine.
However, many dogs live normal lives
and never show any of these symptoms.

Hepatic microvascular dysplasia is a congenital defect,
so Joy was likely born with this condition,
and it went undiagnosed for years. This is particularly troublesome
since Joy was a breeding dog and produced many puppies
that might suffer from the same disease.
It is a condition with symptoms that can be controlled,
but it cannot becured, and common sense (and medical knowledge)
indicate that dogs with HMD should not be bred.

The breeds most commonly affected with HMD are Yorkshire & Cairn Terriers.
Other commonly-affected breeds include Miniature Dachshunds,
Maltese, Miniature Poodles, Shih Tzus, Lhasa Apsos,
Cocker Spaniels, and West Highland White Terriers.

Right now, Joy's condition is managed with three medications
and a special low-protein, liver-friendly diet.
She is scheduled for a liver biopsy, which is the only way
to confirm definitively that she has HMD. If in fact she does,
then the current course of medical management
recommended at Michigan State is the correct one.
We are hoping that after all the tests this poor girl has been subjected to,
a routine of liver-healthy food and necessary medications
will control her symptoms for the rest of her life.

who is a
"foster-to-adopt"
in Donna's home,
will continue to live
her best life with
Donna's family
and her best buddy,
Maxwell.

Sadee continues to be a sweet even-tempered dog.
At this time we are holding our own with eye drops,
but I am still concerned she will lose the left eye.
She is on a specially-compounded extra-strong dose
of tacrolismus eye drops recommended by a veterinary ophthalmologist.

Leroy is a CRUSA Rescue from May 2017. Read his Rescue story HERE

Leroy has gained weight. He is now 40 pounds,
but is not considered overweight by the vet. He loves the snow
and wants me to hand-feed it to him.
He is looking more like a Skye Terrier every day.
He may need to have the left ear canal ablated and probably removal of his left ear.
I am hoping it will not come to that. He has not had an ear infection for seven weeks.
He is on Apoquel, as is Sadee.

Jack is doing well. He gave me a scare before Christmas, but has now rallied.
His last bout of pneumonia was two months ago.
He is playing with some toys and is not as short of breath when he walks.
I do have a stroller when I take him to the vet.
He likes the snow also, but it seems spring and fall will be his best months.
He has gotten a little feisty with his toys. He is on Viagra for pulmonary hypertension
and Benadryl and his symptoms are well-managed.
